Help Me Grow Service Coordinator This position provides in home services, developmental screenings, referral, assessment and support to families of children birth to three who are at risk of developmental delays, who have identified medical conditions which may lead to developmental delays or who have identified developmental delays or disabilities. Reports To The Help Me Grow Service Coordinator reports to the Help Me Grow Supervisor. Employment StatusThis is a full-time position carrying 35-hours per week. Work is regularly scheduled within the hours of 8:30am to 4:30pm Monday thru Friday, except legal holidays, with flexibility of early mornings or evening hours as needed to complete job responsibilities. Some weekend work may be required Principal Responsibilities Task 1 Provide services to families who are enrolled in the Help Me Grow program through home visits and coordination of needed services. A. Meet with parents to determine eligibility, assess family strengths and needs, complete and document all required screenings and develop an Individualized Family Service Plan (IFSP) within required timeline. B. Maintain regular contact with families through scheduled home visits. C. Do initial and ongoing developmental screening on eligible children using the ASQ, ASQ-SE, observation, and parents reports. D. Establish relationships with parents and assist in strengthening the parent- child relationship and improving parenting skills using accepted curriculum. E. Assist parents in improving their parenting skills and child development knowledge to optimize the home environment and assist in the development of social support system using accepted educational materials. F. Aid in establishing a Medical Home as well as providing information on child developmental, children’s health information, immunizations, nutrition information, and referral to appropriate community agencies for additional services. G. Assist families in resolving other issues that affect family life. Other issues might address counseling, substance abuse, OWF requirements, school, housing, budgeting, or transportation. H. Provide transition services and meet transition timelines, to move children at age three, from Help Me Grow to Preschool Special Education or other needed services. Task 2 Develop Individualized Family Service Plans (IFSP) with clients. A. The Service Coordinator will work with parents to identify issues and Goals that the family would like to address and develop an IFSP within 45 days of Referral and in accordance with HMG policy. B. The IFSP will identify activities, services, and individuals needed to accomplish parent goals. C. The IFSP will be written with clear, measurable goals and outcomes. D. The IFSP will be reviewed and updated every 180 days. Task 3 Complete or arrange for assessments and evaluations on children who are suspected of having a delay or disability to determine eligibility for Part C Services, or who are Part C eligible due to a diagnosed medical condition. Complete or revise the IFSP within 45 days. A. Will schedule a home visit with parent to complete the assessment or evaluation of the child that is suspected of having a developmental delay. B. Will assure the completion of the evaluation/assessment. C. Will be responsible for documenting the results of the evaluation/assessment. D. Will document an informed clinical opinion to complete evaluation .E. Will determine eligibility for Part C Services and referral for needed therapies and Early Intervention. Task 4 Complete paperwork and Early Track reporting as required. A. Document case activities through written recordings, IFSPs, correspondence and other required paperwork. Written paperwork must be completed in a timely manner. B. Enter required data into the Early Track system in the required time frame. Task 5 Serve as the agency liaison for Help Me Grow team A. Attend community meetings and events as assigned by supervisor. B. Share information with HMG supervisor and team. C. Represent HMG on Agency committees and functions. Task 6 Participate in regularly scheduled supervisory conferences. A. Present cases for review. B. Share records with supervisor. Task 7 Perform related duties of the position. A. May be required to lead play groups, support groups or other similar activities with Help Me Grow families. B. Give training and presentations upon request within the community. Task 8 Displays professional behavior and ethics. A. Maintain confidentiality of cases within the agency. B. Maintain a professional appearance as required by the agency dress code. C. Maintain professional conduct and manner within and outside the agency, and when interacting with difficult clients. D. Maintain an acceptable attitude regarding being tolerant, patient and non-judgmental with clients, staff, and community. E. Maintain an awareness of own values and their impact on practice. Qualifications Thorough knowledge of child development, specifically birth to three, and knowledge of developmental disabilities. Bachelors degree in social work, family studies, nursing, special education/early childhood education, counseling or related field and experience working with families and children age birth to five. Licensure and experience administering comprehensive developmental assessments preferred. Valid Driver’s License and auto insurance coverage. Equipment Operated IBM compatible computer, Multi-line telephone system, cellular telephone, copier, FAX machine. Position Description This professional position is responsible for the management of human resource and contract management, including procurement-related activities of the agency. Reports To- This position reports to the Deputy Director of Finance and Operations. Employment Status Full-time position. Principal Responsibilities Task 1. Assist in the development, writing, and implementation of personnel policies and procedures. Write, revise, edit, and/or proofread agency policies and procedures and related documents as needed. Prepare and maintain employee handbook and manuals. Stay current on recent federal, state, and case law affecting agency HR. Partner with employees and management to communicate various Human Resource policies, procedures, laws, standards, and government regulations. Incorporate into policy and procedure as needed. Task 2. Participate in staff hiring. Prepare and update position descriptions.Develop recruitment plan and perform recruitment activities. Write and place advertisements. Accept resumes. Reviews applications to match experience and qualifications with specific job related requirements. Coordinates and may conduct interviews. Conducts new employee orientation to ensure employees gain an understanding of agency personnel policies and procedures, as well as benefits plans and enrollment provisions. Task 3. Responds to relation issues such as employee complaints, grievances, harassment allegation, and civil rights complaints. Assure that agency policy and procedures are followed. Collect responses. Make recommendations regarding selection of vendor based upon pre-determined criteria. Oversee preparation of contracts. Task 10 Monitor compliance of agency initiated contracts. Determine if contractor is a subrecipient or vendor. Coordinate with placement providers and contractors to address any issues related to provision of services, contract language or other issues which arise. Perform annual risk assessment. Monitor contractors to ensure compliance with contract deliverables. Develop monitoring procedures for vendors and sub recipients. Make site visits to vendors or sub recipients to ensure compliance with deliverables. Review residential contracts and Individual Child Care Agreements for completeness. Monitor residential and foster home licenses to ensure compliance. Evaluate deliverables, products, and activities. Authorize payments of invoices, based on contract completion and terms. Assure agency contracts are current. Task 11. Oversee contract compliance when agency is provider. Review contracts and work with agency and legal counsel to ensure legal requirements are met. Be familiar with contract deliverables and goals. Ensure that agency staff is in compliance with contract deliverables and goals. Prepare or oversee the preparation of budgets and reports for contracts including budget and program reports. Collect performance data.
